Berryville, county seat of Lee County in eastern Kentucky, has a population of 1700, 98% being white.  The poorest white town in America, it is the third poorest of any American town whether white, black, Latino, or Native American.  Although the sheriff occasionally raids an illegal still, the greater problem is drugs, mostly prescription drugs.  The poor sell goods purchased with welfare vouchers to buy drugs from the needy elderly and from unscrupulous physicians in Florida (the OxyExpress).  OxyContin is ground into power to be ingested or snorted (so-called hillbilly heroin).  Neurontin is also a problem, as is heroin.
